Transfer-in
If you have pension benefits to transfer from a previous pension arrangement download a
Transfer-in (PM5) form.
Estimate of Benefits
You can telephone the Customer Services Team on 0800 2 343434 to get an estimate of your current benefits in the fund or you can download an
Estimate of Benefits (PM70a) form.
Application for Benefits
Fill this form in with your personnel office if you wish to take your benefits.
Download an
Application for Payment of Benefits (PM66) form.
Nomination form
To nominate to the Trustee to whom you would like your death in service lump sum to be paid download a
Nomination (PM6) form You can also use this form to change existing nominations.
